Botulinum toxin is used in various aesthetic and medical procedures to change the facial expression or structure of the facial skin, to eliminate hyperdynamic and static wrinkles.

Botulinum toxin A is the most commonly used preparation for the non-surgical correction of expression lines.

Mimic wrinkles - The first expression lines can form at a young age as a result of repeated emotional facial movements and an innate tendency to move when certain muscles are more active.
Wrinkles are always characterised by an individual expression that varies from person to person. Initially barely visible, these wrinkles become more pronounced and deeper over time. A distinction is made between kinetic wrinkles and hyperkinetic wrinkles.

Kinetic wrinkles - are caused by the movement of the facial muscles, which are directly linked to emotional reactions of the facial muscles and are completely equalised during the resting phase.

Hyperkinetic wrinkles are usually more pronounced on the forehead, nasal bridge and periorbital area (around the eyes), causing an aesthetic imbalance.

Constant, repetitive and too intense activity of the facial muscles is one of the causative factors for the formation of expression lines, which accelerates the involution processes.

These wrinkles are usually visible when the muscles are moving more frequently and are not always associated with pronounced emotions. They even out during the longer resting phase, e.g. during sleep, whereby the wrinkles initially leave soft ‘traces’ that become deeper over time.

Hypertonic wrinkles - Even during the resting phase, the muscles are tense so that they cannot relax completely and do not depend on the person's will.

Static wrinkles - Are caused by degenerative changes in the facial skin and muscular apparatus, which are related to age-related changes in the tissue and are also stimulated by environmental factors (UV radiation, gravity, harmful habits, etc.). These are usually visible during the resting phase, when no movements are made.

How does botulinum toxin work?

The injected botulinum toxin A blocks the neurotransmitter ‘acetylcholine’ at the neuromuscular junction and at the cholinergic ganglia. Without the effect of the nerve impulse, the muscles do not contract. In this way, botulinum toxin A temporarily relaxes the facial muscles by softening or completely eliminating the wrinkles or lines formed by specific muscle contractions.

Are botulinum toxin injections safe?

1987 - a significant milestone in aesthetic medicine. It was the first time that A. and J. Carruthers used botulinum toxin A for cosmetic purposes. Since then, the discovered influence of botulinum toxin has ushered in the era of non-surgical correction of expression lines. Once known as a poison, botulinum toxin is now widely used in aesthetic medicine for the safe and effective correction of age-related skin changes.

However, there are various myths that are still circulating. 

One of them - is botulinum toxin poisonous?

It is not possible to poison yourself with botulinum toxin, as only small doses of the preparation are used. Injections are made into specific muscles that are responsible for the formation of expression lines.  

Is muscle atrophy possible? Muscle atrophy is impossible as the denervation is only temporary and regresses completely within 3-12 months. The normal metabolic processes remain intact in the muscles throughout this period.

Botulinum toxin A is obtained from the bacterium *Clostridium botulinum*. The preparation contains botulinum toxin A in a complex form whose molecular mass favours lower diffusion of the preparation. This means a more precise, predictable result, a longer duration of action and a more natural facial expression.

At what age can botulinum injections be used?

Botulinum toxin is used for aesthetic purposes in people between the ages of 18 and 65. Everyone chooses the time for the first injection based on their own situation and self-reflection.  

For some, the ideal time may be at the age of 20-25, for others at the age of 50. Botulinum injections, which act on the etiological factor of expression lines, are used not only to correct wrinkles, but also for prophylaxis.  

Injections are used from the age of 30-35 or earlier to block involuntary facial expressions and prevent the appearance of the first wrinkles. Wrinkle prophylaxis is also recommended for very intense muscle activity.  

Injections often begin at the age of 30-45, when expression lines intensify due to reduced skin elasticity.  

Third phase - the menopause at the age of 45-55. With the intense reduction of oestrogen and progesterone, collagen and elastin production is reduced, the skin can no longer retain moisture, becomes dry, starts to thin and various wrinkles, including expression lines, become more visible.

Course of the procedure

Usually, the injection area is not anaesthetised before the procedure as the pain is minimal. In case of hypersensitivity, a suitable anaesthetic technique will be offered. Depending on age, gender, depth of wrinkles, localisation and any accompanying individual factors, a small dose of the preparation is injected at the relevant points using a special thin needle. Microinjections block the hyperactive muscles responsible for the formation of expression lines. After the muscle contraction stops, the wrinkles become smoother. The procedure itself only takes a few minutes.

Effectiveness

The effect usually starts to become visible from the 2nd-7th day, but sometimes the effect is only fully visible within 14 days. The facial muscles relax and the skin becomes smoother in the affected areas. The effect lasts for 4 to 6 or even 8 months. Sometimes up to 1 year. Emotions can be expressed as before, but without being accompanied by increased wrinkles on the face.

How often can botulinum toxin injections be performed?

Over time, movement is restored and injections can be repeated. In order to maintain the constant effect, the injection interval is performed every 4, 6 or 8 months. With repeated injections, the effect of botulinum is prolonged up to 6 to 9 or even 10 months. The habit of contorting the face, which favours the formation of expression lines, is sometimes lost, so expression lines can become less intense. It is not recommended to repeat botulinum toxin injections more frequently than every 3 or 4 months.

Recommendations before the procedure

To prevent increased bleeding, the use of non-steroidal anti-inflammatory drugs should be avoided before the procedure.

Recommendations after the procedure

- You can return to your normal routine immediately.  

- Massaging the injected areas should be avoided for 12 hours after the procedure.  

- An upright position should be adopted for 4-5 hours after the procedure.  

- Sports activities, alcohol consumption and sunbathing should be avoided for the first few days after the injection.  

- Overheating (e.g. in hot baths, saunas) should be avoided for 7-10 days after the injections.  

- The use of aminoglycosides and tranquillisers should be avoided in the two weeks following the procedure (the use of these preparations can reduce the effect of botulinum toxin).

When should the procedure be avoided?  

Absolute contraindications:  

  1. diseases with disorders of the transmission of nerve signals (myasthenia, etc.);  
  2. pregnancy;  
  3. breastfeeding;  
  4. local inflammatory processes;  
  5. haemophilia;  
  6. increased sensitivity to the components of the preparation.

Relative contraindications:  

  1. history of kidney problems;  
  2. use of anticoagulants;  
  3. exacerbation of somatic diseases;  
  4. taking medications that influence the effect of botulinum toxin: some types of antibiotics, muscle relaxants.
